Shift: Nights SALARY RANGE: 37.07 - 57.47 HOURLY General

Summary:

The Critical Care Outreach Team RN provides immediate and efficient care for patients at risk for or actively experiencing acute clinical deterioration. This role involves ensuring high standards of patient care and collaborating with other healthcare professionals to improve patient outcomes and safety. This individual will serve as a clinical expert and work closely with critical care teams to execute appropriate, timely interventions.

Responsibilities:
  • Care Coordination:
    Provides prompt and effective response to medical emergencies within the hospital.
  • Coordinate with various departments to facilitate timely and appropriate patient care, including safe transfers to higher levels of care, as appropriate. This includes providing comprehensive critical care interventions in non-ICU settings, as feasible.
  • Mentor and support team members, fostering a collaborative and professional environment.
  • Clinical Expertise:
    Provide expert assessment, clinical care and decision-making for patients experiencing acute deterioration.
  • Perform rapid assessments, initiate appropriate interventions, and monitor patient status.
  • Utilize advanced clinical skills to manage complex and critical situations.
  • Communication and

    Collaboration:

    Serve as a primary point of contact during rapid response events, ensuring clear and effective communication with patients, families, and healthcare providers.
  • Collaborate with physicians, nursing staff, and other healthcare professionals to develop and implement patient care plans.
  • Contribute to case discussions and debriefings, as appropriate.
  • Collects pertinent data information related to assigned duties and impact on patient outcomes.
  • Education and Training:
    Maintain educational requirements, participating in in-services for new equipment, skills and initiatives, as appropriate.
  • Participate in simulation exercises and drills to maintain team readiness and competence.
  • Maintain an understanding of evidence-based practice, clinical guidelines, and advancements in emergency and critical care nursing.
  • All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and

Experience:
  • Education:

    Bachelor Degree Nursing
  • Credential(s):
    Critical Care Registered Nurse (Adult);
    Registered Nurse;
    Advanced Cardiovascular Life Support;
    Basic Life Support;
    Pediatric Advanced Life Support within 6 months of hire; NRP within 6 months of hire, if not already obtained;
    Obtained within 1 year of hire.
  • Experience:

    Minimum of 3 years of clinical nursing experience in a critical care or emergency setting.
Preferred

5 years of clinical nursing experience in a critical care or emergency setting.
