SALARY RANGE: 58.92 - 91.32 HOURLY

General Summary

Health Care Professional responsible for safe, effective, timely medication distribution and administration.

Responsibilities
  • Medication/Solution Order Review – Reviews prescriber’s orders for appropriateness relative to the patient’s condition, height, weight, allergies, and age. Review prescriber’s orders for appropriateness and compliance with legal requirements, organizational policies and procedures.
  • Medication/Solution Order Processing – Utilizes the full functionality which includes: full demographics, allergy review, duplicate therapy, drug-drug interactions, dosage range, and food-drug interaction screening of the automated order entry system to facilitate accurate and timely medication/solution distribution and administration. (Pharmacy Information System Competency).
  • Technician and Ancillary Staff Supervision – Reviews medications prepared for distribution by technicians with respect to accuracy, appropriateness, and timeliness. Monitors technician accountability to follow established policies and procedures for medication delivery for all completed prescriptions.
  • Drug Information Resource – Provides pertinent drug information to medical and ancillary hospital staff. Researches medication dosages, indications, interactions, compatibility, and administration when requested by healthcare professionals. Responds timely and in a professional manner.
  • Medication Therapy Management – To provide Medication Therapy Review by collecting patient‑specific information, assessing medication therapies to identify medication‑related problems, develop a prioritized list of medication‑related problems, and create a plan to resolve them. Pharmacist will acquire certification in Medication Therapy Management within 2 years of employment. To provide Immunization to clients when ordered. Pharmacist will acquire certification in Immunization within the first year of employment.
  • Departmental Performance Improvement Participation – Participates in departmental performance improvement program by ensuring that all quality control measures are followed and properly documented. This includes but is not limited to:
    Overrides, Discrepancies, Near Misses, Oversee Monthly Technician Inspections. Includes documentation requests for technician near misses and identification of medication use scenarios which require submission of Safety First Reports.
  • Education Resource – Conducts staff development programs and provide educational service to pharmacy staff, pharmacy students, residents, clients, nursing staff, physicians, and other healthcare professionals. This accountability includes presentations to the Pharmacy Department, Nursing, Physicians, etc.
  • Intervention Documentation – Documents interventions using the approved documentation tools. (Includes Safety First Reporting, Medication Errors/Near‑miss Reporting.
