Schedule: PRN
Overview: Bring your passion and expertise to our Home Health program. Our Social Workers are the cornerstone of patient-centered care, supporting clients and families in crisis and connecting them to community resources.
What You’ll Do:
- Guide and counsel individuals and families in crisis, ranging from unemployed to those needing foster homes or elderly care.
- Assist clients in obtaining government funds, education, or treatment.
- Secure information—for example, medical, psychological, and social factors—and evaluate client capacities.
- Help patients and families through conferences regarding needs and plans, mobilizing inner capacities and environmental resources.
- Help clients modify attitudes and behavior patterns to improve social functioning.
- Develop a service plan to restore clients to optimum social and health adjustment.

Qualifications:
- Master’s Degree in Social Work
- Five or more years of clinical social work experience
- Valid and current Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) license
- Ability to perform psycho-social assessments and develop and implement viable care plans

Additional Information: The successful candidate will be initially employed by Amedisys until January 1, 2026. After that date the employment will transfer to Adoration Home Health/Bright Spring Health Services.
