Access to Wellbeing Single Point of Access – Band 6 Community Mental Health Practitioner
Access to Wellbeing Single Point of Access Mental Health service invites a qualified practitioner to join our newly formed team. The role will not be based at Kings Park Hospital; the site will be confirmed in due course.
OverviewThe role involves specialist screening, triage, and assessment through telephone and occasional face‑to‑face encounters, supporting referrals, and providing clinical guidance to the multidisciplinary team and VCSE sector. You will contribute to service development, audit, and policy, and support junior staff.
Responsibilities- Provide specialist screening and triage via telephone service within a multi‑disciplinary care team.
- Complete urgent face‑to‑face mental health assessments as required.
- Offer specialist advice on referrals, treatment pathways, and interventions.
- Provide supervision, mentorship, training, and clinical support to colleagues and external partners.
- Deliver a high‑quality, person‑centred approach to care, safeguarding safety, privacy and dignity.
- Execute risk management, signposting, and autonomous clinical decision‑making in line with evidence‑based practice.
- Model Trust values, support change, and contribute to service development.
- Guide and supervise junior staff; promote effective multidisciplinary teamwork.
- Maintain high standards of documentation and data reporting.
- Use resources efficiently, participate in audits, research, and policy development.
- Uphold professional codes of practice, maintain strong IT competence, pursue continuous professional development and challenge poor practice.
• Band 6 Community Mental Health Practitioner level, with advanced, evidence‑based clinical expertise.
• Excellent communication skills for complex and sensitive information.
• Proven experience in telephone‑based triage and occasional face‑to‑face assessment.
• Leadership and supervisory experience; ability to mentor and train others.
• Strong IT and audit skills.
• Commitment to continuous professional development and challenging poor practice.
• Must pass an enhanced Disclosure and Barring Service (DBS) check; the Trust covers the cost.
