Overview

Weymouth ICRT is seeking an experienced Occupational Therapist to join our multidisciplinary team. You will work alongside OT's, physiotherapists, nurses, advanced practitioners, therapy assistant practitioners, rehab assistants and administrative support to deliver intermediate care and rehabilitation to adults in Weymouth and Portland.

• Provide OT‑based rehabilitation for a range of physical presentations including stroke, neurology, orthopaedics, frailty, falls and cognitive impairment.
• Conduct comprehensive cognitive assessments and develop patient‑centred rehab programmes.
• Deliver 7‑day admission‑avoidance support in patients’ own homes and recognise/manage deteriorating patients.
• Carry out basic physical assessments and basic observations, escalates where appropriate.
• Work a rotational rota covering rehab ward duties and community cover until 18:00, including urgent referrals.
• Supervise junior colleagues, manage caseloads autonomously and contribute to service development.

• Current registration with the Health and Care Professions Council (HCPC).
• Experience with moving and handling, use of a range of equipment and cognitive assessments.
• Knowledge of working with older people, frailty and team working in primary and social care sectors.
• Ability to communicate, organise and solve problems independently.
