Advance Practitioner; Nurse

Overview

Great North Medical Group is seeking a passionate and highly committed Advanced Practitioner (Nurse) (ANP) to become a key member of our established and supportive nursing team. This role offers the opportunity to deliver exceptional care within a diverse clinical portfolio, in a forward-thinking, modern, and structured environment. As an integral member of the clinical team, the successful candidate will work autonomously using advanced clinical skills to assess, diagnose and manage patients with a wide range of health needs.

You will make informed clinical decisions, including interpreting diagnostic tests and investigations.

The role includes independently prescribing treatments where qualified and referring patients to other healthcare professionals or specialist services when required. Care will be delivered through face-to-face consultations, telephone assessments and home visits where appropriate. Working within your professional scope of practice, you will provide expert advice to patients, carers and colleagues, maintaining high standards of clinical care and patient safety.

• Triaging and prioritising patient needs, arranging investigations, making timely referrals and managing a defined patient caseload.
• Co-ordinating patient care, including treatment planning, referrals, hospital admissions and discharges.
• Working autonomously using advanced clinical skills to assess, diagnose and manage patients with a wide range of health needs.
• Interpreting diagnostic tests and investigations to inform clinical decisions.
• Prescribing treatments where qualified and referring to other healthcare professionals or specialist services as required.
• Providing expert advice to patients, carers and colleagues to maintain high standards of clinical care and patient safety.
• Adapting duties to service requirements within an annual service contract, demonstrating flexibility in supporting service delivery.

Great North Medical Group is a prominent primary care provider serving the Doncaster area from two main locations:
Chestnut Avenue in Carcroft and Princess Street in Woodlands. Our team includes 9 General Practitioners, 2 Advanced Nurse Practitioners, 7 Nurses, and 4 Healthcare Assistants, collectively caring for around 17,000 patients making us one of the largest medical practices in Doncaster.
