Job Summary

Doncaster North PCN are seeking an experienced Lead Clinical Pharmacist to develop our Primary Care Network pharmacy team. The post holder will work across multiple GP practices in our network delivering advanced clinical medicines optimisation, structured medication reviews, and medicines safety leadership. This is a senior role requiring autonomous practice as an independent prescriber, with responsibility for complex patients, service development, quality improvement and leadership within the multidisciplinary team.

Duties
  • Deliver face-to-face and remote structured medication reviews, focusing on polypharmacy, multimorbidity, care home residents and vulnerable patients.
  • Act as an independent prescriber, implementing prescribing changes and ordering appropriate monitoring.
  • Lead repeat prescribing systems, reauthorisations and medicines optimisation on transfer of care.
  • Manage medicines reconciliation following hospital discharge and reduce medicines‑related admissions.
  • Lead on medicines safety and governance.
  • Drive and deliver quality improvement audit QOF and CQC medicines‑related activity.
  • Support population health initiatives and cost‑effective prescribing programmes.
  • Provide expert medicines advice and education to GPs, MDT colleagues and patients.
  • Support the wider pharmacy team.
  • Act as the designated pharmacy supervisor for trainee pharmacy students.
  • Work collaboratively with PCN leadership, ICB pharmacists and secondary care teams.

Job Responsibilities
  • Lead a team of Clinical Pharmacists, demonstrating role model characteristics and ensuring appropriate clinical supervision.
  • Undertake clinical supervision and systematic peer caseload review of colleagues on an individual or group basis.
  • Ensure all pharmacists are engaged with the review and appraisal systems within the PCN.
  • Act as a role model demonstrating effective clinical leadership, promoting proactive, positive, respectful, supportive, reliable and trustworthy behaviours.
  • Conduct teaching and assessment effectively according to a learning plan with supervision from a more experienced colleague.
  • Manage risk in own and others practice, acting on results to ensure safe delivery of care.
  • Follow professional and organisational policies and procedures relating to performance management.
  • Participate in maintaining systems for risk assessment and minimisation, encouraging a fair blame culture for reporting and investigating near misses and incidents.
  • Contribute to incident and complaint investigations, identifying lessons learned and sharing learning across the organisation.
  • Ensure dignity, privacy and cultural and religious beliefs are respected at all times.
  • Monitor standards of care through quality markers, achievement of PCN targets and benchmarking.
  • Use local quality data to identify practice development requirements.
  • Lead change management in own speciality to improve practice and health outcomes.
  • Identify and provide leadership on areas of prescribing and medicines optimisation, conducting clinical audits and improvement projects.

  • Contribute to national and local research initiatives.
