Purpose of the Role

Social prescribing empowers people to take control of their health and wellbeing. As a Social Prescribing Link Worker (SPLW), you will take a person‑centred, holistic approach to connect individuals to community groups and statutory services, offering practical and emotional support that helps them live healthier, more fulfilling lives.

Key Responsibilities
  • Manage a caseload of people with complex social needs, referred by GPs, Advanced Nurse Practitioners, and First Contact Practitioners.
  • Conduct comprehensive assessments and create personalised care plans.
  • Help individuals identify issues affecting their health and wellbeing.
  • Connect people to non‑medical, community‑based activities and services (e.g., arts, physical activity, nature, specialist advice).
  • Use coaching techniques to empower individuals to take control of their wellbeing.
  • Build strong relationships with local services and community groups, supporting the development of new initiatives.
Knowledge & Experience
  • Experience working within a multidisciplinary team.
  • Background in health, social care, or community development.
  • Ability to collaborate effectively across sectors.
  • Understanding of social determinants of health and long‑term condition management.
Qualifications
  • NVQ Level 3 in Social Prescribing (or equivalent), or willingness to work towards it.
  • Training in health coaching, motivational interviewing, or behaviour change techniques.

  • Excellent communication and rapport‑building skills.

  • Proficient in coaching techniques (goal setting, behaviour change).
  • Strong active listening and trust‑building abilities.

  • Competent in Microsoft Office, web‑based tools, and general IT.
  • Excellent teamwork and ability to integrate into primary care teams.
