Social prescribing empowers people to take control of their health and wellbeing. As a Social Prescribing Link Worker (SPLW), you will take a person‑centred, holistic approach to connect individuals to community groups and statutory services, offering practical and emotional support that helps them live healthier, more fulfilling lives.
Key Responsibilities- Manage a caseload of people with complex social needs, referred by GPs, Advanced Nurse Practitioners, and First Contact Practitioners.
- Conduct comprehensive assessments and create personalised care plans.
- Help individuals identify issues affecting their health and wellbeing.
- Connect people to non‑medical, community‑based activities and services (e.g., arts, physical activity, nature, specialist advice).
- Use coaching techniques to empower individuals to take control of their wellbeing.
- Build strong relationships with local services and community groups, supporting the development of new initiatives.
- Experience working within a multidisciplinary team.
- Background in health, social care, or community development.
- Ability to collaborate effectively across sectors.
- Understanding of social determinants of health and long‑term condition management.
- NVQ Level 3 in Social Prescribing (or equivalent), or willingness to work towards it.
- Training in health coaching, motivational interviewing, or behaviour change techniques.
- Kind, reflective, and focused on what matters to people.
- Empowering, non‑judgmental, and strengths‑based approach.
- Excellent communication and rapport‑building skills.
- Integrity, inclusivity, and openness in all interactions.
- Commitment to diversity, equality, and continuous learning.
- Quality‑driven, promoting high standards of care.
- Proficient in coaching techniques (goal setting, behaviour change).
- Strong active listening and trust‑building abilities.
- Independent decision‑making and problem‑solving skills.
- Competent in Microsoft Office, web‑based tools, and general IT.
- Excellent teamwork and ability to integrate into primary care teams.
