Position: CHILDREN'S CARE MANAGER

Purpose

of the Position

The Care Manager serves youth enrolled in Medicaid with various complex needs, including two or more chorionic conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma. The role covers ages 0-21 (Children’s Program) and 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.

Responsibilities
  • Obtain required Care Management enrollment consents from the medical consenter to process enrollment into the Care Management Program.
  • Complete initial and ongoing needs assessments such as the Comprehensive Assessment, Child Adolescent Needs and Strengths-New York (CANS-NY) and NYS Eligibility Assessment for HARP.
  • Develop and manage the Plan of Care (POC) to coordinate the member’s provision of services, including Care Management Services as identified in the five core services based on the member’s acuity level.
  • Support adherence to treatment recommendations and assist with referrals, ensuring follow-through of referrals.
  • Monitor and evaluate a youth’s needs across prevention, wellness, medical, mental health, care transitions, and social/community services where appropriate.
  • Meet standard billing requirements for each member based on acuity; manage blended caseloads (high, medium and low acuity; adults and children).
  • Provide required face-to-face contacts on a schedule aligned with Department of Health mandates for the member’s acuity level.
  • Ensure Care Management documentation is completed timely and accurately in the daily activity log and electronic health record.
  • Advocate for the member within the agency and with external service providers.
  • Promote wellness by linking youth with resources and services tailored to their needs and preferences.
  • Educate the member/caregiver on chronic conditions, immunizations, screenings and other preventive interventions.
  • Assist the member in obtaining and maintaining public benefits (e.g., Medicaid, Cash Assistance, Social Security, SNAP, housing, legal services, employment, training supports).
  • Communicate with the individual and family with sensitivity to language, literacy, and cultural preferences.
  • Conduct care planning meetings and coordinate services with an interdisciplinary team to deliver holistic care.
  • Identify community resources and manage referrals, access, engagement, follow-up and coordination of services.
  • Engage in discharge planning with hospital admissions to ensure post-discharge services are in place.
  • Participate in ongoing staff development to enhance skills for the Care Manager role.
  • Ensure periodic evaluations and follow-up for dental, vision and hearing care per Medicaid EPSDT guidelines.
  • Develop a close working relationship with the child’s case planner/case manager (if applicable).
  • Ability to travel within 5 boroughs and Nassau and Suffolk Counties.
  • Perform other duties as assigned.
Qualifications
  • Bilingual preferred.
  • Health Home Care Managers serving children with high acuity (per CANS-NY) or Health Home Plus status for adults must have: a) BA/BS with two years of relevant experience, or b) RN license with two years of relevant experience, or c) Master’s with one year of relevant experience.
  • Designated Health Home Care Managers for SED serving children with medium acuity must have at least an associate’s degree with one year of relevant experience.
  • Expertise in servicing youth, adults and families in child welfare, developmental disabilities, mental health, healthcare or related areas; commitment to advocacy.
  • Strong teamwork, communication and leadership skills; ability to work collaboratively and guide peers.
  • Ability to maintain confidentiality and build trusting relationships with peers, members and families.
  • Respect for cultural differences; belief in people’s potential for growth and change.
  • Strong crisis intervention skills and ability to remain calm in crisis; capable of setting boundaries and maintaining a helping role.
Preferred
  • Bachelors degree or better.
