Responsibilities- Provide care appropriate to the patient’s diagnoses, symptoms, and physician orders. Direct and oversee care provided by LPNs and aides.
- Establish and implement a documented plan of care. Determine appropriate outcomes/goals.
- Communicate and collaborate with the patient, interdisciplinary team, and physicians in providing care.
- Promote patient/caregiver education using various verbal and written communication techniques that consider cultural, ethnic, and personal needs or preferences.
- Document patient/caregiver responses to care, treatment, and services, including progress toward goals.
- Identify discharge planning needs early and initiate required referrals to other disciplines, alternative levels of care, or community resources.
- Comply with Joint Commission, federal and state regulations, reimbursement guidelines, and Altru policies and procedures.
- Perform pain assessment and take appropriate action according to standard operating procedures.
- Perform other duties as assigned or needed to meet department/organization needs.

- Must hold an unencumbered nursing license prior to start.
