Transitional Service Coordinator, Volunteers of America-Michigan (VOAMI) is an at-will position that will report directly to the GPD Program Manager in coordination with East SSVF Program Manager. The Transitional Service Coordinator provides stabilization-focused case management to Veterans enrolled in GPD transitional housing and SSVF, ensuring continuity of care from bridge housing through permanent housing placement. The position retains each client up to the first 90 days of SSVF before transitioning them to a long-term case manager or exiting the case if fully stable.
ESSENTIALJOB FUNCTIONS
- Conduct intake, assessment, and rapid stabilization planning for veterans entering GPD.
- Provide intensive short-term case management with a focus on rapid transition to permanent housing.
- This position will be the Primary Service Coordinator for VOAMI GPD clients who are internally referred to SSVF.
- Work independently with veterans who may present with barriers such as mental health needs, substance use, chronic homelessness, or limited independent living skills
- Assist with vital document retrieval, benefits access, employment connections, and essential services.
- Maintain accurate documentation in HMIS and comply with all GPD & SSVF requirements.
- Participate in case conferences and coordination with GPD & SSVF team members.
- Maintain professional boundaries and model appropriate behavior.
- Coordinate housing searches and landlord engagement activities.
- Identify GPD transitional clients ready for permanent housing, with particular attention to those intended for internal SSVF referrals.
- Begin early SSVF readiness preparation for these clients while they remain in GPD, including:
- Document collection
- Income and eligibility verification
- Housing readiness planning
- Provide support with lease signing, utility setup, transportation, and move-in logistics.
- Communicate discharge and move-in timelines to SSVF staff.
- Facilitate a warm, internal handoff from GPD to SSVF with no interruption in case management.
- Conduct all SSVF eligibility screening and enrollment once the veteran secures housing or is SSVF ready.
- Complete assessments, consent forms, and the Housing Stability Plan.
- Provide short-term case management focused on stability, budgeting, rent sustainability, conflict resolution, and community support.
- Maintain compliance with SSVF documentation standards, HIPAA, and agency requirements.
- Coordinate referrals and services with the SSVF team and community partners.
- Maintain accurate and timely record-keeping in HMIS and required platforms.
- Prepare transfer summaries or exit documentation by Day 90.
