M-F, weekends and holidays on occasion, some on‑call.
This position is an outpatient clinic position that assists in the management of a very complex patient population with LVAD. It requires patient follow‑up/assessment in the clinic setting, interrogating devices, following up on labs and test results, following up on patients in rehab, medication refills and prior authorizations, phone calls, and troubleshooting the LVAD device. This position will also require on‑call for patients at home who call with emergent or urgent concerns regarding their device.
It will also periodically participate in the LVAD patient support group.
Ideally looking for RN with a cardiology background.
GENERAL SUMMARY- Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors.
- A coordinator participates in medical, administrative, research and teaching duties.
- The coordinator is an active member of the multi‑disciplinary transplant team.
- Bachelor's degree in nursing preferred.
- Organizational and interpersonal skills to effectively communicate with patient‑care personnel and relate to patients and their families.
- Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.
- Analytical ability and motivation necessary to effectively plan, deliver, and implement plans of care in acute and outpatient situations.
- For those working as a Coordinator of Heart Transplant/with VADS:
Experience in ICU or OR preferred.
Current RN license in the state of Michigan.
