Role DescriptionThis is a part‑time RN Nurse Case Manager based in the Detroit Metropolitan Area. This position could grow to full time, if the applicant were interested in doing so. The RN Nurse Case Manager will be responsible for overseeing and coordinating care plans, discharge planning, and performing utilization management to optimize patient health outcomes. The position involves collaborating with healthcare teams, patients, and their families to ensure effective and efficient care transitions while meeting patient‑specific needs.
Responsibilities- Attend and coordinate doctor appointments as needed
- Coordinate DME, home health care, and therapies as needed
- Effectively communicate to medical providers and family as needed
- Effectively communicate with insurance representatives
- Conduct rate surveys on services and/or medical equipment
- Obtain relevant medical documentation for the claim
- Assist in reviewing and setting up necessary vendors
- Coordinating client medications and supplies
- Coordinating with therapies and vendors for home and/or vehicle modifications
- Valid Driver License
- Must be able to pass a background check
- Excellent organizational and problem‑solving skills to manage and prioritize multiple cases
- Strong communication and interpersonal skills to interact with patients, families, and healthcare teams
- Registered Nurse (RN) licensure required
- Case management certification (CCM or similar) is a plus
