RN Case Manager

Work Arrangement: Fully Remote — no onsite work required

License: RN State License

The Case Manager RN serves as the primary point of contact for members, coordinating holistic care through telephonic and digital channels. Using the case management process, this role develops and manages individualized care plans to improve health outcomes and ensure continuity of care across the healthcare continuum.

• Coordinate care with a multidisciplinary team (Social Workers, Dietitians, Pharmacists, etc.).
• Assess member needs and create personalized care plans.
• Close gaps in care and address barriers impacting health outcomes.
• Educate members on health literacy, medications, and self-management.
• Support transitions of care, including discharge planning and resource coordination.
• Document all interactions and meet quality, productivity, and compliance standards.

• Nursing Diploma or ADN required;
  BSN preferred
• 3+ years clinical nursing experience (acute, post-acute, or community)
• 1+ year case management in managed care preferred
• Experience supporting members via phone and digital platforms preferred
• CCM required within 18 months of hire (CCP preferred)
• Strong critical thinking, time-management, and communication skills
• Proficiency in Microsoft Office & remote collaboration tools
