Overview
This position is an outpatient clinic position that assists in management of a very complex patient population with LVAD. It requires patient follow up/assessment in the clinic setting, interrogating devices, follow up on labs and test results, follow up on patients in rehab, medication refills and prior authorizations, phone calls and trouble-shooting the LVAD device. This position will also require on call for patients at home who call with emergent or urgent concerns regarding their device.
This position will also participate periodically in the LVAD patient support group. Ideally looking for RN with cardiology background. Working collaboratively with a highly specialized team. Great opportunity to establish long term professional relationships. M- F, weekends and holidays on occasion, some on- call.
- Interrogate LVAD devices and monitor device-related data.
- Provide patient follow up and assessment in the clinic setting.
- Follow up on labs and test results; manage medications, refills, and prior authorizations.
- Coordinate care for patients in rehab and facilitate on-call support for home patients with emergent or urgent device concerns.
- Participate in LVAD patient support group.
- Collaborate with a highly specialized multidisciplinary team.
- Bachelor's degree in nursing preferred.
- Strong organizational and interpersonal skills to communicate with patient care personnel and relate to patients and families.
- Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.
- Analytical ability and motivation to plan, deliver, and implement care plans in acute and outpatient situations.
- For those working as a Coordinator of Heart Transplant/with VADs:
Experience in ICU or OR preferred.
Current RN license in the State of Michigan.
