Role Summary

We are seeking an experienced and dedicated Acute Care Inpatient Case Manager to join our team. This role is responsible for coordinating patient care, managing utilization review, and ensuring safe and efficient transitions of care for patients admitted to the hospital. The Case Manager will serve as a vital link between patients, families, physicians, and the healthcare team to promote quality outcomes and cost-effective care.

Duties and Responsibilities
  • Conduct comprehensive assessments of patients’ medical, psychosocial, and discharge needs.
  • Collaborate with physicians, nurses, and interdisciplinary teams to develop individualized care plans.
  • Perform utilization reviews to ensure medical necessity and compliance with regulatory requirements.
  • Coordinate discharge planning, including arranging post-acute services, equipment, and follow-up care.
  • Advocate for patients and families, addressing barriers to care and facilitating access to resources.
  • Monitor patient progress and outcomes, adjusting care plans as needed.
  • Maintain accurate documentation and comply with hospital policies, payer guidelines, and accreditation standards.
  • Other duties as assigned.
Required Qualifications
  • Current Registered Nurse (RN) license or equivalent clinical licensure.
  • Minimum 5 years of inpatient acute care case management experience.
  • Strong knowledge of utilization management, discharge planning, and care coordination.
  • Excellent communication, critical thinking, and problem-solving skills.
  • Proficiency in EMR systems and case management software.
Desired Qualifications
  • Certification in Case Management (CCM or ACM) preferred.
  • Bachelor of Science in Nursing (BSN) or higher degree.
  • Experience with diverse patient populations and complex care needs.
Location & Work Type

Full-time, onsite

Type: full-time contract employment 13-14 weeks with possibility to extend.
