Overview
The Nurse Practitioner (NP) role is part-time and focuses on providing primary and chronic care to elderly patients within the PACE program.
Responsibilities- Deliver primary, acute, and chronic care in clinic, home, and facility settings.
- Collaborate with the interdisciplinary team and under the Medical Director's guidance.
- Conduct assessments, develop care plans, manage medications, and coordinate care transitions.
- Participate in team meetings and educate patients and families on health topics.
- Accurately document in the EHR and support quality initiatives.
- Master’s degree in Nursing
- Active NP license
- 2+ years in geriatrics
- Experience in hospice preferred
- Passion for senior care
The role involves travel within multiple counties, treating participants both in the center and at home.
Benefits$75,000/year with flexible schedule.
