Overview
Ascension St. John Hospital in Detroit, MI is seeking a dedicated and experienced Registered Nurse Case Manager (RN CM) for a travel assignment. The Case Manager will play a critical role in coordinating patient care, ensuring appropriate utilization of resources, and facilitating transitions across the continuum of care.
Responsibilities

• Conduct comprehensive assessments of patients to determine care needs and discharge planning requirements

• Collaborate with physicians, nurses, social workers, and other healthcare professionals to develop individualized care plans

• Monitor patient progress and adjust care plans as needed to achieve optimal outcomes

• Ensure compliance with hospital policies, regulatory requirements, and accreditation standards

• Advocate for patients and families to ensure access to appropriate services and resources

• Facilitate communication between patients, families, and the healthcare team

• Document case management activities accurately and timely in the electronic medical record
Qualifications

• Current Registered Nurse (RN) license in the state of Michigan (or compact license eligible)

• Minimum of 2 years of recent case management experience in an acute care hospital setting

• Strong knowledge of discharge planning, utilization review, and care coordination

• Excellent communication, organizational, and problem-solving skills

• Ability to work independently and collaboratively in a fast-paced environment

• BLS certification required; CCM certification preferred
Assignment Details

• Contract length: typically 13 weeks, with potential for extension

• Monday–Friday

• Shift: 8:00 AM – 4:30 PM
