POSITION
PURPOSE:
The Registered Nurse is a dedicated and compassionate candidate who will be responsible for conducting health screenings and providing health education to patients. This role involves working closely with individuals to promote wellness and preventive care through comprehensive assessments and, when necessary, referrals.
- Current Registered Nurse (RN) license in the state.
- Experience with Federally Qualified Health Centers (FQHC).
- Minimum of 2 years of nursing experience, preferably in public health or community health settings.
- Strong communication and interpersonal skills to effectively interact with patients and their families.
- Ability to work independently and as part of a team in a fast-paced environment.
- Certification in Basic Life Support (BLS) and other relevant certifications is a plus.
- Commitment to ongoing professional development and education.
- Conduct health screenings for blood pressure, diabetes, cholesterol, and other chronic conditions, ensuring accurate measurements and documentation.
- Administer immunizations according to established guidelines and protocols, maintaining up-to-date records.
- Perform and coordinate mammogram referrals, providing education on breast health and the importance of early detection.
- Educate patients on lifestyle modifications, nutrition, exercise, and disease prevention to promote overall health and well-being.
- Assess and document patient medical histories and health status, identifying any potential health risks.
- Collaborate with other healthcare professionals to develop and implement individualized care plans.
- Maintain a clean, safe, and organized work environment, adhering to infection control and safety protocols.
- Stay informed on current health trends and best practices to provide the highest quality of care to patients.
- Participate in community outreach programs to promote health awareness and preventive care services.
- Must be able to work a flexible schedule, including some evenings and weekends.
- Required to work closely with patients and with the Clinical Practice Manager and Community Health Workers.
- Exposure to communicable diseases and unhealthy populations, as well as disturbing and uncomfortable issues or matters contrary to personal beliefs.
- Demonstrated ability to work effectively as a team with staff from various areas of responsibility.
- Ability to learn Electronic Medical Record and State reporting systems.
