The Case Manager provides support and advocacy to clients and their families within the community to educate and promote healthy lifestyles and preventing reoccurring hospitalizations by monitoring, assessing, advocating, supporting and empowering members to improve their quality of life through maintaining stability in the community.
Responsibilities- Conduct initial and annual assessments.
- Provide population and age specific case management services to members.
- Assess the personal needs of each member.
- Participate with members in the development of the Individual Plan of Service.
- Provide support to members and families in identifying and accessing resources.
- Maintain required case management documentation.
- Participate in in-service training, clinical supervision and consultations.
- Participate in group meetings related to substance abuse and mental health.
- Assist in providing education to non-CMH providers regarding mental health.
- Assist in the development of community contacts and strengthen networks.
- Bachelor’s degree in human services required.
- One year of experience in working with a person who is exhibiting mental health challenges.
- Valid Michigan driver’s license required.
- Must complete requirements for a Michigan Certified Board of Addiction.
- Must be willing to complete requirements for Professionals of MBAP.
- Basic Life Support (BLS) certification required.
For intake and case management services:
Qualified Mental Health Professional (QMHP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field.
