Clinical Counselor - Full Time Days

Essential Functions
  • Conduct psychosocial assessments and develop effective treatment plans that promote patient safety
  • Gather information from patient observation, interviews with patients and families to inform treatment planning
  • Provide crisis intervention and assist families in understanding implications and complexities of life situations
  • Participate in multidisciplinary treatment team meetings to discuss assessments, treatment planning and discharge planning
  • Identify, assess and coordinate patient cases and related needs across illness, hospitalization, finances, education and community services
  • Facilitate education, process groups, support groups and referrals as requested
  • Conduct intakes and assist in coordinating placements within facility programs
  • Develop and execute clinically relevant activities in coordination with activity professionals
  • Promote recovery, symptom reduction and highest level of social functioning
  • Communicate with and support family members
  • Manage a caseload and ensure accurate patient chart documentation
  • Maintain documentation to meet facility and accreditation standards
  • Supervise social work interns as required
  • Provide direction and guidance to social services staff as required
  • Lead the program/unit and coordinate activities of other clinical or direct patient care staff
  • Maintain a positive, empathetic, and professional attitude; address complaints and prioritize patient safety
  • Perform other duties as assigned
Education/Experience/Skill Requirements
  • Master's degree in Social Work from an accredited college or university required
  • Three or more years' related experience working with a comparable patient population
  • Supervisory experience preferred if the role includes supervising staff
Licenses/Designations/Certifications
  • State licensure as required by the state; LLMSW, LMSW, LLPC, LPC
  • CPR and de-escalation/restraint certification required (training available on hire and provided by the facility)
  • First aid may be required based on state or facility
