Overview

Location: Teen Hype Youth Development Program - Detroit, MI 48207

Position Overview: The Youth & Family Community Health Worker connects Teen HYPE youth and families to health, mental health, and social resources. This non-clinical role focuses on resource navigation, advocacy, health education, and family support using a Healing Centered Engagement approach. This role requires someone who knows Detroit's resource landscape deeply, understands the systemic barriers families face, and can advocate effectively to ensure youth get the services they need.

Services are youth-centered and family-focused, delivered during time frames when youth can access them.

Responsibilities Resource Navigation & Advocacy
  • Serve as the primary point of contact for youth and families seeking health and social services
  • Assess needs and connect families to community resources through warm handoffs, scheduling, and follow-up
  • Track referrals, troubleshoot barriers, and advocate for youth and families when access issues arise
  • Build and maintain partnerships with health providers, schools, and community agencies
Health Promotion, Education, Family Engagement & Support
  • Facilitate healing circles, peer support groups, and wellness activities for youth
  • Provide one-on-one health coaching and age-appropriate health education
  • Support youth in accessing primary care and establishing medical homes
  • Assist families with health insurance enrollment and preventive care navigation
  • Coordinate health screenings and wellness events with community partners
  • Build trusting relationships with parents and caregivers
  • Provide guidance navigating healthcare, insurance, and social service systems
  • Connect families to resources such as food assistance, housing support, and emergency services
  • Conduct home visits and facilitate family meetings when needed
Staff & Community Support
  • Support staff wellness through healing circles and trauma-informed practices
  • Train staff on recognizing health needs and making effective referrals
  • Share resource guides, updates, and best practices with staff and partners
Qualifications

Required

  • Bachelor’s degree in a related field
  • 2+ years working with youth and families
  • Knowledge of Detroit community resources
  • Strong communication and relationship-building skills
  • Valid driver’s license and reliable transportation
  • Flexible schedule (some evenings/weekends)

Preferred

  • Community Health Worker certification
  • Lived experience in Detroit
  • Trauma-informed or healing-centered experience
  • BLS / CPR First Aid certification (or willingness to obtain within 60 days of hire)
Salary & Employment

Salary: $45,760.00 - $56,160.00 (based on experience & qualifications)

Employment Type: Full-Time
