Integrated Case Manager – Population Health
Setting: Ambulatory, Inpatient, or Health Plan
Position Type: Contract / Travel
The Integrated Case Manager for Population Health is a key member of the patient-centered care team, responsible for coordinating and managing care for patients with complex medical, behavioral, and social needs. This role ensures safe, high-quality, and cost-effective care across the entire continuum, including ambulatory, inpatient, and health plan environments.
The Case Manager supports patients who have experienced critical events or complex diagnoses requiring intensive coordination, resource utilization, and advocacy.
Key ResponsibilitiesClinical & Care Coordination
- Conduct comprehensive assessments of patient and caregiver needs (medical, psychological, social, and functional).
- Develop personalized, patient-centered care plans.
- Engage patients and families through education, advocacy, and ongoing communication.
- Use motivational interviewing, clinical judgment, and self-management techniques to overcome care barriers.
- Provide counseling related to treatment decisions, end-of-life planning, and Advance Care Planning.
- Coordinate care transitions between ambulatory, inpatient, post-acute, and community settings.
- Ensure services are delivered appropriately within the patient’s health plan benefits.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
- Facilitate care conferences for complex patients.
- Continually evaluate and modify care plans based on patient response.
Population Health & Safety
- Identify and address social determinants of health impacting outcomes.
- Facilitate interventions for:
- Child abuse and neglect
- Domestic violence
- Elder and institutional abuse
- Sexual assault
- Connect patients to community and social service resources.
Quality & Process Improvement
- Participate in process improvement initiatives focused on quality, cost, and patient safety.
- Support department and organizational goals.
Education
- Nursing Degree
OR - Master of Social Work (MSW)
Licensure
- Active Michigan RN License (unrestricted)
OR - Active Michigan LMSW License (unrestricted)
- Strong verbal and written communication
- Excellent customer service and interpersonal skills
- Strong critical thinking, problem-solving, and decision-making
- Strong computer and documentation skills
- Experience preferred in:
- Discharge planning
- Home health
- Rehabilitation
- Community health
- Managed care
- Knowledge of:
- Medicare & Medicaid
- Case management principles
- Clinical practice & preventive care guidelines
- Behavior change theory
- Medical ethics and legal compliance
- Social determinants of health
