Pay range – $17 to $25 per hour based on experience

We are seeking an experienced and compassionate Doula/Community Health Worker to provide emotional and physical support to expectant mothers and their families. The Doula assists mothers through learning about birth preferences during prenatal consultations, advocating for clients during and after labor and delivery, and assisting with newborn care following births. This role also provides educational support to expecting parents, as well as assistance with breastfeeding and caring for a newborn.

• Conduct prenatal consultations to explore clients’ birthing preferences and discuss any concerns
• Create birthing plans that outline the delivery room or home birth setting, who will be present, and pain relief options
• Assist with preparations for the child’s arrival, such as setting up the nursery or running errands
• Provide physical, emotional, and informational support to birthing individuals and their families during labor and while recovering
• Advocate for clients to ensure medical providers adhere to their wishes
• Provide advice about nursing and assist with childcare during the early postpartum period
• Stay up-to-date with the latest research on childbirth, prenatal care, and infant development
• Collaborate with primary care team members to ensure comprehensive client support
• Address social determinants of health and eliminate barriers to care as appropriate

• Overall Outreach and Tracking – document outreach attempts in the EMR, provide monthly reporting to Health Plans, and coordinate education and engagement activities
• Patient and Community Engagement – schedule appointments, coordinate outreach programs, and engage high‑risk patients in care management
• Health Risk Assessment – assist patients in completing assessments and address triggered interventions
• Emergency Department Utilization – encourage engagement with PCPs and address barriers that lead to inappropriate ED use
• Gaps in Care – prioritize patient outreach to close gaps in preventive services and chronic disease management
• Community Resources/Social Determinants of Health – conduct SDOH screenings and connect patients with community resources
• Administrative Functions – maintain accurate documentation, submit forms on time, maintain confidentiality, and attend staff meetings
• Supervisory and Training – participate in supervisory meetings, training sessions, and complete the CHW training program within 180 days of hire

• Well‑organized, detail‑oriented, and capable of multi‑tasking in a dynamic environment
• Strong commitment to serving underserved and vulnerable populations
• Excellent interpersonal communication, active listening, and advocacy skills
• Ability to navigate the healthcare system, maintain confidentiality, and adapt to changing community needs
• Experience in community work, education, or health care is strongly recommended

• Certified Doula through a state‑approved training program
• High School Diploma or equivalent
• Excellent communication and time‑management skills
