Overview

Join our award-winning team at OakPointe Senior Care & Rehab Center. The Social Worker provides social services to residents and their families to help them cope with illness and extended care placement. This position participates in resident care management and ensures needs are met through Social Service programming and appropriate referrals to outside agencies and community resources. What you will need to become part of our team
  • Degree in Social Work, or in related human services fields (including Sociology, Special Education, Rehabilitation Counseling, Psychology) preferred but not required
  • Licensure in Social Work preferred but not required
  • 1–2 years of supervised social work experience in a healthcare setting
  • Excellent customer service with a commitment to delivering exceptional, compassionate care
  • Team player who supports colleagues and helps get the job done
How you will help people every day
  • Identify medically-related social service needs of residents and ensure they are addressed by the appropriate discipline
  • Maintain contact with family (with resident’s permission) to report on changes in behavior, psychosocial functioning, discharge planning, and care-planning participation
  • Assist residents/responsible parties with financial and legal matters as needed
  • Coordinate discharge planning services, including waiting lists for community placement and arranging home-care services
  • Identify and support residents’ individual needs, preferences, routines, concerns, and choices during assessment and care planning
