Registered Nurse Clinical Team Manager

Your mission is to lead, coordinate, and optimize the delivery of home health services while maintaining the highest standards of clinical quality and regulatory compliance.

Responsibilities
  • Provide oversight of all patient care services and personnel, including assignment of patients and staff, coordination of referrals, and ongoing assessment of patient needs.
  • Plan, direct, and participate in the delivery of home health services to provide diagnosis‑specific care, coordinating services with providers, vendors, and facilities.
  • Ensure timely start of patient care in accordance with regulatory guidelines.
  • Process and coordinate physician orders, staff assignments, and communications between the direct care team, patients, families, and physicians.
  • Review and approve care plans; evaluate proposed changes for clinical appropriateness.
  • Integrate orders from all involved physicians and confirm that approvals are obtained.
  • Conduct regular OASIS reviews for all patients.
  • Hold regular meetings with direct care staff to provide guidance, coaching, and communicate organizational changes.
  • Initiate hiring requests, interview candidates, and recommend hiring of direct care staff.
  • Communicate with the Branch Director or Administrator on key performance metrics.
  • Remain available for field work as needed in a physically demanding, high‑stress environment.
  • Perform other duties/projects as assigned.

Requirements
  • Associate degree in a nursing‑related field (required)
  • Minimum 2 years of home‑care experience as a registered nurse in a Home Health or Hospice environment (required)
  • One year of supervisory experience (preferred)
  • Current, unrestricted RN license valid for the state of employment (required)
  • Proficiency in enterprise applications such as Home Care Homebase, Workday, and Microsoft Office
  • Willingness to travel within the branch/office coverage area
  • Dependent vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 lbs

You will report to the Administrator or Branch Director.
