Case Manager - St.Dominic Program Des Plaines, IL

Job Summary:

The Case Manager (Residential) at the St. Dominic Program in Des Plaines, IL, works under the supervision of the Case Management Supervisor to advocate for youth, supporting them in exercising their rights, choices, and responsibilities. This role involves monitoring program development and treatment planning to ensure a safe and nurturing environment for the youth. The position requires advanced knowledge in the field of child welfare, with a primary focus on discretion and judgment.

Responsibilities
  • Provide face-to-face social services to youth in accordance with the Individual Treatment Plan
  • Complete comprehensive assessments and develop service plans for assigned youth
  • Prepare court reports, attend court hearings, and follow court orders related to youth services
  • Facilitate youth intake processes and transfer school records and employment history
  • Connect youth with community resources and provide necessary linkage to available resources
  • Monitor and address youth's medical, dental needs, and checkups
  • Maintain organized case files with all essential information
  • Help youth set and achieve personal, educational, social, and employment goals
  • Participate in clinical staffings, meetings, and training sessions
  • Document case management activities and complete Medicaid billing for prescribed services
  • Ensure timely completion of Quality Utilization Reviews and required forms for DCFS
  • Coordinate discharge planning and transition services for youth
  • Participate in performance quality improvement initiatives
  • Perform other duties as assigned
Qualifications
  • Bachelor’s degree in a human service-related field
  • Minimum of one year of residential care experience
  • Minimum age of 25 years
Physical Demands
  • Ability to sit, stand, bend, stoop, and walk for extended periods
  • Capability to lift/move objects weighing over 20 pounds
  • Visual, hearing, and learning capabilities to perform essential job functions
Environment and Scheduling
  • Work occurs in a designated setting
  • Potential interaction with challenging youth
  • Flexibility to work additional hours, weekends, and holidays as needed
