Registered Nurse - Community Living Center

Position: Registered Nurse - Community Living Center 3
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Delivers fundamental knowledge-based care to assigned clients at a Community Living Center. Assesses, plans, implements, and evaluates restorative nursing care. Administers medications and procedures while coordinating care focused on patient education and collaboration with the interdisciplinary team.

* Completes Restorative Nursing assessments and develops individualized care plans

* Completes Restorative Nursing sections of the Minimum Data Set (MDS) documentation

* Administers medications and treatments prescribed by licensed healthcare providers

* Provides health teaching, counseling, and care supportive to patient well-being and illness prevention

* Conducts chart audits and compiles data for Performance Improvement (PI) indicators

* Provides training and education to disciplines regarding the Restorative Nursing Program

* Develops action plans to correct identified deficiencies in care delivery

* Educates older adults and families about chronic illness and mental health treatment options

* Assists in coordinating care among caregivers and interdisciplinary team members

* Participates in quality care improvement activities and Unit Based Council initiatives

Pay:
Competitive salary, regular salary increases, potential for performance awards
