ResponsibilitiesThe Weekend Package RN schedule is every weekend (Saturday & Sunday) from 7:00AM – 8:00PM, as well as four (4) hours on Mondays to conduct follow-ups with providers and/or complete documentation. This is a full-time, exempt (salary) position. The Weekend RN partners with the Clinical Team to administer and coordinate skilled nursing care to patients requiring home care services. Services are provided in accordance with physician’s orders, under the direction and supervision of the Clinical Director, and in compliance with applicable laws, regulations, and policies of the organization.
What You’ll Do
- Clinical Care:
Provide direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Maintain and update schedules of all disciplines in patient’s home. - Assess and evaluate patient status and goals; revise the plan of care as necessary and communicate adjustments to the team in a timely manner. Complete authorization modifications as needed.
- Initiate and apply preventative, therapeutic, and rehabilitative procedures; maintain communication regarding patient care with the physician, supervisory personnel, and other professional staff involved in patient care.
- Monitor and manage patient medications; maintain an accurate medication profile; coordinate prescription refills with physicians and pharmacies.
- Teach the patient and family/caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the physician. Offer health counseling opportunities with patients and families/caregivers.
- Provide and maintain a safe environment for the patient. Report unsafe conditions, complaints, and incidents on behalf of the patient as appropriate.
- Observe and supervise Home Health Aide & LPN to evaluate performance and quality of care.
- Accept responsibility for specialized procedures, such as IV therapy, when qualified with appropriate training and proven competency.
- Use supplies and equipment effectively and efficiently; maintain equipment per agency policy.
- Fulfill the obligation of assigned patient case load including tracking patient status when transferred, following up with other facilities, and resuming care as appropriate.
- Adhere to HIPAA laws and maintain patient confidentiality at all times.
- Other duties assigned by the case manager/clinical management, e.g., performing field supervisions of care team members as required by agency policies.
Documentation
- Complete and submit accurate, timely clinical notes including all OASIS documents, visit authorization requests, physician orders, and all other clinical documentation regarding patient’s condition and care provided in accordance with Innovive company policies.
Communication and Meetings
- Communicate with physicians and other agencies providing nursing or related services on a consistent basis to ensure continuity of care and implementation of a comprehensive care plan. Act as part of the interdisciplinary care team.
- Participate in scheduled clinical and internal/external case management meetings. Attend vendor and referral meetings as needed or as patient conditions require. Document all meetings and outcomes for patient medical record.
