About the Role:

Registered Nurse Case Manager
As a Registered Nurse Case Manager, you’ll play a key role in supporting the health, happiness, and daily experience of the residents at Wesley at Home.

• You will have the opportunity to directly and positively impact the health and wellbeing of the people you serve through the provision of professional nursing services.
• Support a case load of hospice patients in an assigned area.
• Provide compassionate, person-directed care to patients and families.

• Full-Time Monday–Friday, Back-up on-call Weekdays and weekends

• Valid, unrestricted Iowa driver’s license.
• Current vehicle insurance.
• Current license as a registered nurse in the state of Iowa.
• Two years of full-time nursing experience required, with one year of hospice experience preferred.

• The pay range for this registered nurse position is $75,025 - $90,603 / year. The starting pay rate will be based on years of experience.
