Job Details
- Must have a valid nursing license with verification dated within 30 days of start.
- Must complete Trinity Specialty Specific Competency Testing within the past year.
- Must provide documentation of physical/health statement within the last year indicating fitness to work.
- Must complete Trinity Code of Conduct Form, Trinity I9, Trinity Confidentiality of Patient & Proprietary Information and Information System Security Agreement, Trinity Integrity and Audit Services Form, Trinity Non Employee Upload Form, Trinity Conflict Of Interest Disclosure Form, Trinity Criminal Background Check, Trinity Education Verification, Trinity EPLS, Trinity GSA/OIG, Trinity Joint Commission Testing, Trinity NSO, Trinity System Access Form, Trinity Skills Checklist, Trinity Child and Adult Dependent Abuse Training (Iowa only), Trinity IA Sing Report (Iowa only), and Trinity OFAC within required time frames.
Job Requirements
- Every Other Week (EOW) schedule.
Schedule Information
- Must complete Fit Test.
- Must complete Trinity Color Vision test.
- Must have COVID-19 vaccine proof with booster by September 2025 or declination/exemption (declination not accepted for CA or IL).
- Must have MMR vaccination proof or IgG titer.
- Must have Hepatitis B vaccination series started prior to start date.
- Must have Influenza vaccine proof or declination/exemption for flu season 10/1/2024-4/30/2025 and 10/1/2025-4/30/2026.
- Must have 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements.
- Must have Varicella vaccination proof or IgG titer.
- Must have Tdap vaccine or declination if no pediatric interaction.
