Palliative Nurse RN

Responsibilities- Deliver compassionate, comprehensive care by educating patients, managing medications, assessing health, and providing emotional support to patients and families.
- Coordinate interdisciplinary care plans by collaborating with nurse practitioners, social workers, and other professionals, while triaging patient needs and guiding advance care planning.
- Ensure seamless transitions and compliance by supporting intake and referrals, maintaining accurate documentation, and facilitating hospice care when appropriate.
- Travel in all weather conditions to patients homes within the assigned territory.
- Valid, unrestricted Iowa driver’s license.
- Current vehicle insurance.
- Current license as a registered nurse in the state of Iowa.
- Two years of full‑time nursing experience required, with one year of hospice experience preferred.
- The pay range for this registered nurse position is $75,025 - $90,603 per year. The starting pay rate will be based on years of experience.
