Overview
Host Home Providers needed in the Northwest Iowa region. This is a contracted, independent provider role (independent contractor) with LSI (Lutheran Services in Iowa).
Responsibilities- Provide guidance, support, and care for the individual living in your home.
- Work with the mentee on goals such as shopping, budgeting, coping skills, cooking meals, personal hygiene, and community integration.
- Act as an advocate/mentor to help the person live their best life possible.
- Establish a meaningful relationship with the person hosted and integrate them into your family environment.
- Coordinate with LSI for matching, training resources, and ongoing support.
- Complete required trainings and a home safety check prior to matching and placement.
Northwest Region of Iowa, encompassing 33 counties (including Story, Hardin, Franklin, Cerro Gordo, and others listed in the original posting).
Requirements- Must be 21 years or older
- Must reside in Iowa
- Must have a high school diploma or GED
- Must have a current/valid driver’s license
- Meet minimum insurance requirements
- Have access to a computer/internet to complete documentation requirements
- Successfully complete all steps of the screening, interview, and matching process before a mentee begins receiving support
- Be flexible and creative
- Have a passion for making a difference
Additional requirements include a valid driver’s license, auto insurance, commercial general liability insurance, a decent driving record, and the ability to pass extensive background checks (including checks of Dependent Adult and Child Abuse registries, Sex Offender registry, criminal history, references, and a home safety check). Background checks are also required for all residents over 18 in the home.
