Base Pay Range$80,000.00/yr - $90,000.00/yr

The autonomous Licensed Clinician role lets clinicians function like a private practice without the hassle of managing a back office. Ability to work within a 40‑hour work week; majority of time clinically focused, remainder patient documentation and facility correspondence.
On‑site clinical provider working with residents and their families, active lead member on interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.

Responsible for initial assessments, psychotherapy to address mental health issues and offer ongoing support, and all documentation requirements per Medicare & commercial insurance plans.
Key Requirements- Entrepreneurial spirit, strong medical knowledge, documentation proficiency (CMS, Medicare, Medicaid), and nursing home/hospital facility experience.
- Master’s degree and a valid LISW/LMFT/LCPC license in the state of application.
- Two years counseling and therapy experience.
- Team player with positive attitude.
- Demonstrated knowledge of mental health diagnoses and therapeutic approaches.
- Experience collaborating with interdisciplinary teams, including other MPAC providers.
- Open communication with facility team and Regional Clinical Manager to ensure success of clinical programs.
- Initiative and effective communication in multiple settings (patient, family, nursing staff, social services, other providers).
- High organizational level to manage patient needs, documentation, and ensure mental health needs met.
- Comfortable with ever‑changing environment of skilled nursing facility.
- Knowledge of Medicare Benefit and commercial insurance plans.
- Experience in a healthcare setting.
- Passion for working with older adults.
- Exceptional organizational skills.
- Experience in nursing home space is a plus.
