The Clinical Coordinator is responsible for coordinating the daily programming of the Partial Hospital Program (PHP). The Coordinator organizes daily staff schedules with input from clinicians and medical staff, responds to referrals, and coordinates the admission of individual patients into the PHP. LCSW preferred.
Responsibilities- Perform clinical roles relevant to the individual’s clinical experience.
- Organize daily programming schedule for patients and staff.
- Organize admissions into the program.
- Maintain Patient Tracker – track referrals, admissions and declinations.
- Correspond with insurance companies for referrals and/or pre‑certifications as needed.
- Conduct concurrent reviews with insurance companies as necessary.
- Attend and participate in weekly PHP staff meetings.
- Lead and participate in weekly Treatment Team meetings.
- Participate in weekly clinical supervision.
- Work with the interdisciplinary team to meet treatment goals.
- Maintain employee time schedules and approve time in Kronos.
- Enter daily charges for patients’ group attendance.
- Update daily patient attendance/census.
- Complete patient registration process.
- Conduct tours of the unit to prospective patients and their families.
- Assist in maintaining social media communication (Facebook, Instagram, blogging, website inquiries).
- Assist with ongoing outreach to referral sources.
- Assist with maintaining monthly statistics around referrals and synthesizing data into useful outreach opportunities.
- Master’s degree in Social Work from an accredited school.
- Completion of CPI Nonviolent Crisis Intervention within 30 days of hire.
- LCSW preferred.
- Two to four years in social services or case management preferred.
Located in Derry, NH, Parkland Medical Center is an 86‑bed regional acute‑care hospital and a designated Level III Trauma Center. Salary range: $50,000.00 – $60,000.00.
