Registered Nurse; RN), MDS Coordinator

A leading Skilled Nursing & Rehabilitation facility in Denville, NJ is seeking an experienced Registered Nurse (RN) MDS Coordinator for a full-time role with the opportunity to transition into a permanent position.

This role is ideal for a clinically strong, highly organized RN who excels in assessment, documentation, and coordination across interdisciplinary teams.

Requirements

• Active New Jersey Registered Nurse (RN) license
• Minimum 1 year of experience as an MDS Coordinator
• Strong understanding of RAI process, MDS 3.0, and PDPM
• Proven accuracy in assessment, coding, and documentation
• Ability to meet regulatory timelines and submission deadlines
• Strong communication and collaboration skills
• Competence in care plan development and interdisciplinary reviews

Preferred Requirements

• Experience in long-term care or sub-acute rehab
• Familiarity with care plan software (e.g., Point Click Care  or similar)
• Knowledge of quality measures and QAPI initiatives
• Previous leadership or team coordination experience
• Experience with PDPM reimbursement optimization strategies
