- American Heart Association or Red Cross certifications accepted.

- Primary Source Verification of professional license and/or certification required within 30 days of start.
- Must score 80% or higher on Prime Basic Pharmacology Exam.
- Must score 80% or higher on Prime Basic Rhythm Interpretation Screening.
- 2 COVID-19 vaccines and booster required or declination completed.
- Proof of 2 Vaccines or Positive Titer showing immunity for MMR.
- Annual Respirator fit test required.
- Hepatitis B vaccination required.
- Flu Vaccine or declination required November 1st to March 31st.
- TB Test required within year of start date.
- Driver's license, State , or Passport required.
- Vaccination/titer showing immunity for Varicella required.
- Tdap vaccination required.
- Physical/Health Statement required annually.
- National Criminal Background Check within one year of start date.
- Prime Agency Attestation required.
- Prime SAM/EPLS check required.
- JPEG photo for badge required.
- Prime Form I-9 (Employment eligibility verification) required.
- Must score 80% or higher on Prime Core Competencies Exam.
- Prime Skills Checklist required.
- Prime Color-blind test required annually.
- Prime Criminal Background Check required.
- Prime Specialty Exam required.
- Prime Orientation Packet required.
- Prime Job Description signed by employee required.
- Prime Education Verification required.
- DCI/BVP certification required.
- Prime Drug Screening (minimum 10-panel test) within 60 days of start required.
- Resume up to date and accurate required at time of offer acceptance.
- 2 references required, one from a manager or direct supervisor.
- Name Verification Affidavit required for any name discrepancies between credentials.
