FLSA Status:
Full-Time, Non-Exempt

Pay Range: $24.00 - $26.00 per hour

Position Summary

This position is a dual-role split equally between grant-funded support services and agency-wide intake coordination. The employee will spend 20 hours/week providing "Trauma-Informed Supportive Services" to unhoused individuals as defined by the Arapahoe County grant, and 20 hours/week managing the agency's intake and VI-SPDAT assessments.

Key Responsibilities
  • Grant-Funded Support Services (50% - Arapahoe County)
    • Target Population:
      Provide trauma-informed supportive services specifically to unhoused individuals in Metro Denver.
    • Direct Service:
      Maintain a caseload of grant-eligible participants, creating stability plans and navigating barriers to housing.
    • Grant Reporting:
      Track and document specific data points required for the Arapahoe County Quarterly Reports, including:
      • Number of Arapahoe County residents served.
      • Program impact measures and demographic data.
      • Use of funds summaries.
    • Deadlines:
      Ensure all data is entered and ready for review prior to quarterly submission dates (April 15, July 15, Oct 15, Jan 15).
  • Agency Intake & VI-SPDAT (50% - Empowerment Program)
    • Intake Coordination:
      Serve as the first point of contact for new referrals, conducting initial screenings and safety assessments.
    • VI-SPDAT Administration:
      Perform the VI-SPDAT (Vulnerability Index - Service Prioritization Decision Assistance Tool) to assess vulnerability and prioritize housing placements.
    • Documentation:
      Ensure all intake paperwork is accurate and uploaded to the database within 24 hours of the appointment.
    • Referral Management:
      Route eligible clients to the appropriate internal program (e.g., Housing, Employment, Therapy) based on assessment outcomes.
  • Qualifications
    • Education:
      Bachelor's degree in Human Services, Social Work, or related field (preferred).
    • Experience: 2+ years of case management experience.
    • Required Skills:
      • Proficiency in VI-SPDAT administration.
      • Experience with data entry for grant reporting.
      • Ability to work with unhoused populations using a trauma-informed approach.
