Position Overview
The Empowerment Program is seeking a dedicated Housing Care Manager to support our Continuum of Care (CoC) project. This grant-funded position is primarily supported by the HUD CoC "Supportive Services" budget allocation.
The Housing Care Manager will provide comprehensive recovery support services and housing stability management for program participants. This role is responsible for ensuring compliance with HUD regulations (24 CFR part 578) and helping participants maintain permanent housing.
Key Responsibilities- Conduct housing resource searches and landlord engagement to secure units for participants.
- Process annual recertifications to verify participant eligibility and income in accordance with 24 CFR part 578.
- Deliver Supportive Services to promote long-term housing stability and recovery.
- Develop individualized service plans focusing on self-sufficiency, recovery support, and connection to community resources.
- Assist participants in navigating public benefits, healthcare, and employment resources.
- Maintain regular contact with participants to monitor progress and address housing barriers (e.g., lease violations, utility arrears).
- Maintain precise participant files documenting eligibility, homelessness status, and income verifications.
- Ensure all services provided comply with federal anti-discrimination laws and immigration verification requirements (PRWORA/SAVE).
- Document all financial assistance and service encounters in the Electronic Health Record.
- Experience:
Minimum 2 years of experience in case management, permanent supportive housing, or homeless services. Familiarity with HUD CoC grants preferred. - Knowledge:
Understanding of the Unhoused population and recovery-oriented systems of care. - Education:
Bachelor's degree in Social Work, Human Services, or a related field is preferred. Relevant lived experience or professional experience in homeless services may substitute for formal education year-for-year. - Skills:
- Trauma-Informed Care & Harm Reduction:
Demonstrated experience applying Trauma-Informed Care and Harm Reduction principles is essential. The role involves working with vulnerable populations (e.g., those with substance use disorders or criminal justice involvement), requiring a non-judgmental approach to foster housing stability. - Motivational Interviewing:
Proficiency in Motivational Interviewing (MI) techniques to support clients in setting self-directed recovery goals and maintaining lease compliance. - Crisis De-escalation:
Proven ability to manage interpersonal conflict and de-escalate crises, which is critical for maintaining safety in a residential or community-based setting.

- Requirements:
Must be able to pass background checks and verify eligibility to work on federal grants. - Valid Driver's License: A valid Colorado driver's license, reliable vehicle, and insurance are required for conducting home visits, transporting clients to appointments, and meeting with landlords across the Denver metro area.
