This position supports the mission of Presbyterian/St Luke’s Medical Center of optimizing quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. Within scope, the nurse navigator oversees and directs the care of the BMT or Hematology-Oncology patient and is responsible for collaboration with all disciplines to provide well-coordinated patient care.

This position functions to navigate blood cancer patients through the continuum of care including diagnosis, treatment, follow-up and long-term care needs. This includes coordination of chemotherapy orders, prescriptions and individualized follow-up. The hematology nurse coordinator demonstrates competence in the ability to triage patients over the phone and in person. The hematology nurse coordinator demonstrates the ability to interpret laboratory and radiologic test results and communicates results to physician as necessary.
