The FCS Clinical Manager is a highly qualified healthcare provider who, through education and experience, possesses a distinct body of knowledge and skills and applies the art and science of his/her profession.

The FCS Clinical Manager is in charge of helping implement Family Caregiver Services from a clinical standpoint and supporting the RN Care Managers in the field. He/she applies nursing knowledge and critical thinking in assessing, implementing, and evaluating client responses to actual or potential health problems. He/she is responsible for coordinating clinical activities and supporting the healthcare team to provide the highest quality of client care.

He/she will be able to assist with all clinical duties, including but not limited to skilled assessments to determine eligibility, start of care, supervision of CNA’s, IHSS Attendants, and/or LPN’s, and recertification, transfer, and discharge visits. The FCS Clinical Manager works under supervision of and receives clinical direction from the Director of Nursing. The FCS Clinical Manager will also help with administrative tasks such as, but not limited to coordination of services, contacting physician offices, requesting outstanding documentation, and review of completeness of records.

At COFK, the FCS Clinical Manager (RN) is expected to be proficient and efficient with the use of an EMR system and multiple technological platforms to provide the highest quality of care to the patients and families we serve.

  • Director of Nursing

Education/

Licenses/Certifications:

  • Bachelor’s Degree in Nursing and two years experience in home care specifically surrounding CNA Program and IHSS Program.
  • Licensed as an RN in the State of Colorado.
  • BLS CPR certification.
  • Specific specialty experience may be required for certain populations/areas.

Experience/Requirements:

  • Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and legal documents.
  • Ability to read and interpret physicians’ orders and notes from other caregivers as well as respond to them verbally and in writing.
  • Ability to effectively present information to clients and employees and community.
  • Ability to respond effectively to the most sensitive inquiries or complaints.
  • Ability to gauge distances.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to deal with several abstract and concrete variables. Ability to improvise in a home setting.
  • Ability to send/receive emails and use the internet for research and informational purposes.
  • Ability to use independent judgment to manage and impart confidential information.
  • Skill to attend to multiple tasks at once which are time sensitive.
  • Basic knowledge of Windows operating system and ability to advance computer skills as required.
  • Ability to establish and maintain effective working relationships with fellow employees, the public, and supervisors.
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability or financial condition of the client.
  • Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff and interact professionally as a member of a multidisciplinary team.
  • Must possess a strong commitment to the goals, mission and philosophy of the organization.
  • Ability to work flexible hours as required to meet identified client needs.
  • Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments.
  • Ability to adapt to changing organizational needs.
