Weekend (every other) Triage Hospice Nurse - Registered Nurse (RN)
We are seeking an RN to work every other weekend from 8:00 am to 5:30 pm on both Saturday and Sunday. This could be a 30 hour/week role with additional hours seeing patients.

- Provide care and services for a group of hospice patients
- Visit patients where they reside, including private homes, assisted living facilities, and hospitals
- Complete hospice admissions and patient visits as needed
- Answer calls from patients, families, and customers
- Make nursing visits for symptom management, deaths, and admissions as needed
- Work independently and on your own on a day‑to‑day basis with activities including phone, email, and written communication
- Current RN licensure in good standing in the state(s) of practice with one year of experience under license
- Admissions and on‑call experience preferred
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Can work independently and on your own day‑to‑day with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations
- Time management skills to manage visits, documentation, and meetings (schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical record system
- Willingly drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area
