Position: Weekend (every other) Triage Hospice Nurse - Registered Nurse (RN)

Weekend (every other) Triage Hospice Nurse - Registered Nurse (RN)

We are seeking an RN to work every other weekend from 8:00 am to 5:30 pm on both Saturday and Sunday. This could be a 30 hour/week role with additional hours seeing patients.

Responsibilities
  • Provide care and services for a group of hospice patients
  • Visit patients where they reside, including private homes, assisted living facilities, and hospitals
  • Complete hospice admissions and patient visits as needed
  • Answer calls from patients, families, and customers
  • Make nursing visits for symptom management, deaths, and admissions as needed
  • Work independently and on your own on a day‑to‑day basis with activities including phone, email, and written communication
Qualifications
  • Current RN licensure in good standing in the state(s) of practice with one year of experience under license
  • Admissions and on‑call experience preferred
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Can work independently and on your own day‑to‑day with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations
  • Time management skills to manage visits, documentation, and meetings (schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
  • Ability to learn an electronic medical record system
  • Willingly drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area
