Overview
Job Description: The Behavioral Health Coach, guided by the treatment team's direction and supervision, delivers direct and impactful therapeutic interventions to patients and their families, empowering them to achieve their treatment goals. This role collaborates with clinicians, patients, families, healthcare providers, insurers, community resources, and all relevant stakeholders to deliver comprehensive services, coordinate care, develop effective transition plans, and advocate for patients' well-being.
Responsibilities- Works under identified licensed mental health professional and actively participate in required supervision through scheduled meetings, consultations, and chart reviews as indicated.
- Demonstrates expertise to role-model trauma-informed care to establish strong patient relationships, interpret behaviors, and respond effectively to patient needs.
- Collaborates with clinicians to develop, model, and implement interventions for patients with moderate to severe behavioral and emotional problems.
- Facilitates interventions with patients and families to promote goal setting, skill building, co-regulation, and family education in line with the care plan.
- Builds strong therapeutic rapport to enhance treatment engagement, using behavioral health expertise. Identifies, anticipates, and diffuses crisis situations; works to manage difficult patient behaviors and ensures safety. Covers on-call responsibilities in accordance with program standards and rotation schedule.
- Observes, documents and reports patient behaviors, engagement, and organization to the treatment team to inform patient care strategies. Completes timely and accurate documentation compliant with all regulatory and procedural requirements in the medical record.

- Satisfies all Intermountain Health requirements regarding procedural, operational, and compliance training and updates. Performs other duties as assigned.

- Bachelor’s degree in social work, psychology, sociology, family studies, or related field (education is verified) OR four years of experience working in a related field.
- Excellent written and verbal communication skills
- Proficiency in computer applications
- Positive driving motor vehicle (DMV) record, proof of current automobile insurance coverage, and a copy of a valid and current Colorado driver s license
- Basic Life Support and First Aid within 90 days of employment
- Experience working in a behavioral health setting
- Bi-lingual in Spanish
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- Ability to regularly lift and / or move up to 50 pounds. Employees must be physically able to use crisis prevention intervention techniques including: physical management of a child, running to prevent a runaway incident. While performing the duties of this job, the employee is frequently required to stand, walk, sit, reach with hands and arms; climb or balance; stoop, kneel, crouch, run or crawl.
