** Successful candidates:
** Bachelor’s Degree in Social Work, Psychology, Human Services or related field - MSW preferred2 years social work experience in a health care setting working directly with individuals.

Experience in a post-acute or skilled nursing facility or hospice required!
** Rate**: **$65,000-$75,000/year
