Overview

Uptown Community Health Center cares for over 9,000 patients across 4 clinics;
Bruner Family Medicine, Caritas Internal Medicine, Seton Women's Center, and Uptown Midwifery Center.

Summary

Provides a safe and collaborative therapeutic environment to assess patient motivation and readiness to change and assist patients in developing skills and behaviors to enhance functioning to the highest level of independence. Maintains a safe and therapeutic environment through the use of various individual and group treatment modalities.

Essential Functions
  • Upholds the Mission, Vision, and Values of Uptown Community Health Center
  • Maintains a safe, secure, and healthy environment by adhering to organization standards, policies, and procedures, and to legal regulations; alerting others with potential concerns.
  • Communicates in a professional, clear, effective, and compassionate way with patients, visitors, and coworkers. Both in person, on the telephone, and with written communication.
  • Evaluates and treats patients within the licensed scope of practice; appropriately refers patients for specialty evaluation and treatment; provides preventive health advice and case management; and consults with other staff.
  • Provide care to patients with any / all types of substance use disorders.
  • Assess coping mechanisms and current functioning in the following areas: life skills, social skills identity, interpersonal, family, and group dynamics.
  • Plan and develop group and individual treatments as a multi-disciplinary team member, including physicians/providers.
  • Participates in multi-disciplinary, team-based care of patients
  • Implement group and individual treatments, ensuring a safe, comfortable therapeutic environment.
  • Ensure the follow-through of treatment goals concerning individual and group therapy.
  • Prepare documentation of the patient's progress and response to treatment in a timely manner.
  • Document patients' progress and response to treatment in a timely manner.
  • Participates in training, reporting, and other requirements of various grant-supported programs.
  • Proper and accurate documentation within the electronic medical record: recording patient examination, treatment, and test results.
  • Maintains patient confidentiality and protects medical office operations by keeping information confidential; cautioning others regarding potential breaches.
  • Maintains a clean work area:
    Clean and stock the rooms in accordance with the Kanban system.
  • Participate in huddles daily, per huddle procedure.
  • Attend required staff meetings, standard work meetings, trainings.

Licensure/Qualifications/Experience

Required

  • Licensed Addiction Counselor - for Colorado
  • BLS Certification
  • Master's or Doctorate degree in a clinical behavioral health field (e.g., social work, psychology, counseling) from an accredited institution
  • Two (2) years of relevant experience

Preferred

  • Five (5) years of experience in a related field
  • Experience with Epic application and health record
  • Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
  • Bilingual English / Spanish, complete comprehension, literacy, and fluency in both languages
