• Provide personal care assistance including bathing, dressing, and grooming.
• Give medication reminders.
• Assist with daily living activities and mobility support.
• Engage clients in social and recreational activities to enhance well‑being.
• Maintain a clean and safe environment for clients.
• Prepare meals and assist with feeding.
• Follow the Care Plan and document care activities reporting any changes in client condition.
• Maintain client safety and report changes in condition.
• Collaborate with HCA Manager to ensure continuity of care.
• Provide transportation for appointments and activities.
• Perform light housekeeping and laundry.
• Clock in and clock out.
• Build trusting relationships with clients and their families.

• High school diploma or equivalent;
  State certification and license in good standing.
• Previous experience in caregiving or home health care is a plus.
• Familiar with Well Sky Personal Care app preferred.
• Strong communication skills and a compassionate demeanor.
• Ability to follow care plans and maintain client confidentiality.
• Current CPR and First Aid certification is a plus.
• Reliable transportation and a valid driver's license.
• Willingness to work flexible hours, including weekends and holidays.
• Must pass background checks and drug screening.

• Ability to lift, push or pull up to 25 pounds.
• Ability to assist with mobility and physical transfers.
• Ability to stand, bend and kneel for extended periods.
