Oncology Stem Cell Transplant LCSW

Full Time with rotating Sundays and rotating on call Saturdays.

Responsibilities
  • Work with inpatient and outpatient multi-disciplinary oncology team to identify and meet the needs of oncology patients as they progress from diagnosis through survivorship.
  • Thoroughly assess the psychological, social, cultural, and financial needs that may impact patients undergoing transplant.
  • Utilize counseling and clinical knowledge to make recommendations and provide appropriate support for patients and families through the transplant process.
  • Demonstrate strong verbal and written communication with providers, patients, families, and other members of the care team, and resolve conflicts creatively related to psychosocial needs.
  • Conduct discharge planning assessments and make recommendations and referrals to post‑acute facilities to achieve optimal patient outcomes.
  • Provide counseling in individual and support group settings for patients and caregivers during and after treatment.
  • Assist patients with applications and documentation for work leave of absences, grants, and/or referrals for assistance applying for SSDI, Medicaid, or COBRA.
  • Identify appropriate timing for end‑of‑life discussions and involve palliative and hospice care resources.
  • Engage in ongoing education related to BMT/Oncology Social Work and pursue professional certifications.

Qualifications
  • Master of Social Work (MSW) from an accredited school of social work.
  • Licensed Clinical Social Worker (LCSW) in the State of Colorado.
  • 3–5 years of recent clinical social work experience in a healthcare setting; experience working with oncology patients preferred.
