Position: Licensed Vocational Nurse LVN / LPN

Overview

Trio Home Health (Jet Health, Inc) is looking for PRN / Per Diem Licensed Practical Nurses (LPN / LVN) to join our dedicated team to provide community‑based care for our patients currently receiving home health services in Denton County, TX and surrounding areas.

The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Responsibilities & Qualifications
  • Graduate of an accredited School of Vocational/Practical Nursing.
  • Current State license as a Licensed Vocational/Practical Nurse, or a multi‑state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.
  • Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
  • HCHB experience highly preferred
  • Skills:

    Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
  • Reliable transportation and valid, current auto liability insurance.
  • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary group, patient/family, and referring agency.
  • Assist in the development and implementation of patient care plans.
  • Monitor patient health and report any changes to the supervising RN.
  • Administer medications and treatments as prescribed.
  • Educate patients and their families on health care needs, conditions, and options.
  • Maintain accurate and timely documentation of patient care and progress.
  • Work in patients' homes in various conditions.
  • Ability to work a flexible schedule and travel locally.
  • Ability to lift up to 50 lbs and move patients.
  • Requires working under some stressful conditions to meet deadlines and patient needs.
  • Ability to make quick decisions and acquire necessary resources.
  • Must meet patient/family individualized psychosocial needs.
  • Requires hand-eye coordination and manual dexterity.
