Overview
Trio Home Health & Hospice / Jet Health, Inc. is seeking a compassionate and skilled Registered Nurse to join our team PRN / Per diem completing admissions for our new Home Health patients in the Denton, TX and surrounding areas.
The primary functions of this role are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume the responsibility for coordination of care.
Responsibilities- Perform admissions for home health patients in patients homes.
- Administer nursing care and conduct assessments for patients of all ages in the home setting.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Develop and document the plan of care in correlation with the assessment.
- Provide nursing care utilizing infection control measures (OSHA).
- Work with the Interdisciplinary Group to ensure patient/family needs are met.
- Communicate with attending physicians, home health physicians, and other physicians involved in the patient’s care.
- Ensure timely submission of all required paperwork.
- Meet productivity standards and participate in on-call responsibilities as assigned.
- Provide effective communication to patient/family, team members, and other healthcare professionals.
- Travel locally and work a flexible schedule; PRN emergency call availability.
- Education:
Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing. - Licensure:
Current state license as a Registered Nurse. Proof of current CPR certification. Current Driver's License. - Experience:
Two years’ experience as a Registered Nurse in a clinical setting; one-year full-time home health experience preferred but not required. - Knowledge:
Knowledge of OASIS highly preferred but not required. - Skills:
Strong assessment and clinical skills, excellent communication, ability to work independently and as part of a team, familiarity with infection control measures (OSHA), knowledge of state and federal regulations, good interpersonal skills. - Home Care Home Base (HCHB) knowledge highly preferred.
- Transportation:
Reliable transportation and valid driver s license; valid and current auto liability insurance. - Ability to work a flexible schedule and travel locally.
