Care Transition Specialist – Care Management

Work location:

Texas Health Denton, 3000 North I-35, Denton, TX 76201

Work hours:

Full time;
Monday – Friday, 8:00am – 4:30pm

Texas Health Denton is a Joint Commission-accredited Primary Stroke Center and Chest Pain Center and a Bariatric Surgery Center of Excellence and a Pathways to Excellence-designated hospital. We are the only choice in Denton for oncology services, women and infants’ services, and much more.

What You Need
  • H.S. Diploma or Equivalent required
  • Three years’ relevant experience, preferably in a healthcare or service environment required
  • Patient Care/ Customer Service / Healthcare experience strongly preferred
  • MS Outlook, MS Office (Word, Excel), internet navigation, clinically based software, and financially based software
What You Will Do
  • Responsible for coordinating the clerical and administrative processes for entity care transitions management
  • Responsible for managing departmental mailbox, phone lines and/or fax
  • Supports transition planning activities as needed under the direction of care transition managers
  • Calls vendors, physician offices, clinics, etc.
  • Faxes (electronic and traditional) documents as needed
  • Makes copies or gathers other data as needed
  • Collect and enter data as needed
  • Completes other tasks and projects assigned
